pequenas novidades - some little news

Adoro ouvir o risinho dele!

A few days ago Artur figured out he could play alone and have fun. He found out that his hands are quite funny, he already played with them a long time ago but he didn't laugh. Now sometimes I hear him laughing and giggling while he's playing alone with his hands. I just love to hear him laughing!

Ferias - vacation

We had really great vacations this year! In the beginning of August Artur had 2 doctor appointments, one because of the hearing aids and the other was a follow up with the genetics doctor. We were by the beach almost all month  and Artur really liked the sea water even though it was really cold. The first day we went to the beach he didn't like to much too feel the sand but after a while he got used to it. We bought a sort of backpack to go with Artur to the beach without using his baby carriage (?), he loved it and every time he was there he fell asleep lulled by the motion of our steps. Well, about the appointments, the first one didn't go to well 'cause Artur didn't cooperate while they tried to test him with the hearing aids on, but they try it again and if it fails again they'll do a different test. The Genetics appointment went really well, the doctor did the follow up and really like the development that Artur has achieved, its true that they develop slowly but as long as they keep having some developments and achieve new goals all is good because that's the most important thing! :P

Artur @ Bairro Alto!!!

Saturday we went to Bairro Alto with some friends to have diner. We took Artur with us, so with 2 years 4 months and 6 days old he went to the most known place of Lisbon night life. He captured the attention of a drunk Rastafarian that said something we didn't get that sounded like "Magui ungui something Man!" after a while we saw him again and he waved goodbye to Artur. To celebrate Artur's first time in Bairro Alto we bought there this baby bottle.

Artur

Yesterday Artur started his baby talk with the hearing aids on. Since Wednesday he only did his baby talk when we took the hearing aids off. Still doesn't laugh with them on, but he gives us a big smile when we make him laugh. Still gets some scares with loud noises but we think he is doing really well in getting used to the hearing aids. We noticed that he is more annoyed but maybe its because his world became louder than he was used to. It was 2 years in a more silence world, everything is louder now so it must be hard for him.

Aparelhos Auditivos - hearing aids

Artur got the hearing aids! He got them last Wednesday, he is in adapting mode... still gets some scares with sneezes and doors closing. Started the Speech Therapy this Thursday. I'll put some photos of his hearing aids and Artur with them on.

desenvolvimento ou aprendizagem? - development or learning?

Today I decided to potty train Artur. He doesn't sit unattended but I was holding him. Since he poops only when I change his diaper at night with the diaper opened, I decided to buy a chamber-pot (?). Well after not even 5 minutes sitting down he did it! It wasn't at the usual time of day that he does it! I just tried it because I was going to change the diaper and wanted to know how he felt sitting on it. My doubt is: Is this a sign of development, is it just learning a new skill or was it just plain luck?

Boas notícias - Good news!

Hi everyone!
Yesterday we went to a meeting in a organisation where we live to evaluate Artur so we could get or not the Speech therapy. The meeting was so natural it seemed like an informal talk with friends. I gave them all the necessary documents and reports of the times he was on the hospital. At the end, with no delays they gave us the answer: Artur is going to start the Speech therapy! We scheduled an hour and the speech therapist will be in our place one time a week for an hour session!

Today after PT we went to the audiologist to do the molds for Artur's hearing aids. He was really tired and when we waited for our turn he ended up falling asleep. After about 10 minutes we were called in to do the molds. The lady that did them explained me everything and even answered a few questions I had about his hearing aids. When we left they scheduled the next appointment for next month but they will try to see Artur as soon as they have a vacancy. The rest of the text is about how they were so nice and great and so I decided to publish the company name: WIDEX, they are in other countries also and if they are as good as in here they certainly are a great place to consider! ;)

Exame auditivo - hearing exams

Yesterday Artur did new hear exams, for the first time, he was sleeping during the all exam. It went really well. Today I got a call from the place that will make his hearing aids. We will go there next Monday to do the molds. Artur needs  Speech Therapy and so it starts the search for a therapist.

Aparelhos auditivos - Hearing Aids

Like all followers and readers of this blog know, Artur is waiting for hearing aids since last year. It's been a year and almost one month since I delivered the 3 budgets for consideration by the Purchasing Management of the Hospital. This past Wednesday, I went to the Hospital's Users Support (?), to try to know how I could know how was Artur's request for hearing aids. I tried to speak with the social worker who said that only had time for me one hour later, and advised me to go directly Purchasing Management to know the status of Artur's request. After some waiting, (1st to know the request number and then to speak with the colleague of the first lady), I was informed that the process was still locked. What does this mean? When I delivered the 3 budgets, was assigned a request number and (probably) they did not analyze the request. I explained that Arthur has two years now, and that in his ENT appointment, the doctor said it was urgent to get the hearing aids for Artur cause the lack of them could and would certainly delay the talk skills and cognitive development, because if we do not hear well we can not process the right way in our brain the information we receive. Now with two years he does not speak (only says 4 words when we wants too), so the speech is already affected by lack of the hearing aids . (I know that speech can also be affected by the disease of Artur, but of course if he does not hear well how can he talk? How can I assess what is because of the lack of hearing and what is because of the disease itself?)
The lady who spoke to me was very understanding and said she would talk with her superiors to try to unlock the process of Arthur since he is a 2 year old boy. But she said there were no guaranties and stated that there are no funds for purchasing technical aids. Got out of there with a sinking heart thinking: Even if they unlock the process, who knows when he will get the hearing aids?
I went to Artur's physiotherapy, and on the way home I received a call from the lady She was so excited and told me she had great news, they unlocked Artur's request! She explained that now they had to see if there was funding for the purchase, said to me what was the chosen place and asked for my email so that if there was money she could send a copy of the order form. She said that only next week I would hear from them if there was funds, and that if there was she would send an email with the order form or call me to say there was no funds. I thought to myself will all these excitement does she knows already that they will give finally the hearing aids to Artur and is not to telling me that because its all because I was there? Then I thought maybe she is happy because she could get to unlock Artur's request.
I know it is already a long story, but this morning I peek at the e-mail and surprise! I received an email from the lady with the order form! Those who know me will not wonder what I will say, but I was in tears! A mixture of joy with a touch of anger since it took one year for Artur to receive what he needs! Right now, got the bitter taste of thinking: If I had gone sooner would he already have the hearing aids? Probably not but now how to silence the doubt inside my head?

I am very happy! Now, we are waiting to be contacted by the chosen place, the hospital has chosen the best one, the one they told me that's best for children because they have specialists in children audiology, (very important fact because most places do not have specialists in audiology for children).

Exames auditivos - Hearing tests

Yesterday Artur went to do some hearing tests, to re-evaluate his situation, cause as we already know he has hearing loss in both ears. The tympanogram was a bummer, and he didn't want to do it. So he ended up all upset and little he let them do afterwards. The little they could test they realized he is better because he react to lower sounds than the last time he did this test. So now he has to do a test he has done last year: the potentials evoked (?).

Novidades - news

Today Artur went to his physiatry appointment. He spent most part of the time bitting his shoes. lol
He has better head control and has a better muscle tonus in the upper body (where he has the hypotonia).
Such good news! He is getting better slowly. (He's on baby steps speed. lol)
Tomorrow he is going to do some hearing tests, lets hope for good news as well!

hoje - today

Artur a brincar com o pé - Artur playing with his foot

Today it has not been a good day for Artur. He has fever, maybe because there's 2 teeth coming out. Because of the fever he is having small seizures (less than 2 minutes each). While the fever last he will continue to have seizures cause he is really sensible to the temperatures. Lets see if he has a good night sleep.

Artigo do DN - newspaper article

Article translation (in progress):
in the "balls" are some of the features of the disease

Summary:
1p36 deletion  is the name of this health problem as complex as its symptoms. Motor delay, hearing and vision problems, mood explosions or epilepsy are just some of the consequences of this "orphan" disease, which is part of the list of nearly 700 registered in the country. When her son was six month old Raquel knew that he suffered from a genetic problem that has no cure. But that can be fought with treatment and monitoring

title:
The rare disease with no cure that stops Artur's development

Article:

Artur is 2 years old, but he doesn't sit up or crawl, he just rolls over. His last major win was to be able to hold his head, what happens around 5 month old for other children. Artur has almost no speech. He says mommy, hi, no, and am when he is hungry. Artur is one of two children in Portugal with a rare disease: 1p36 deletion. The name is as complex as all that the disease causes. It is a genetic problem caused by the loss of genetic material on the short arm of chromosome 1, which affects the normal development of children. After confirming the diagnosis of the child, Raquel and her husband ran tests that showed no genetic abnormality, which proves the rule in other cases known to the world: they are the parents who transmit the disease to children. Although 1p36 is rare is still "the deletion [loss of a segment of chromosome] more often found" confirmed to the DN Juliette Dupont, medical genetics at the Hospital Santa Maria, Lisbon, who made the diagnosis when Artur was just six months. The frequency, he says, is one in every five thousand babies. But the president of the National Association of Mental Disability and Rare diseases (Raríssimas), Paula Brito e Costa, confirms there is only one more case in the country. In Portugal they are known at least 680 rare disorders, those that reach one person in two thousand. The diagnosis of 1p36 requires a genetic test. Raquel, 31, told DN that since he was born, the child showed symptoms that trouble the doctors. Was seen by clinicians from various specialties, until Juliette Dupont received the test result (it is only necessary to collect blood from the patient). The mother immediately sought information about the disease. She tried to react quickly and be prepared for everything. Even for the worst. Artur's disease is not degenerative, but has several problems associated with the organs. "The Artur is healthy in that aspect," she said, adding that expect a long life for her child.  However, Arthur has epilepsy and hearing problems (waiting for hearing aids) and will conduct tests to confirm if the vision is also affected. He suffers the most common symptoms of disease: a motor delay. Because of these limitations, it is accompanied by a physical therapist and a physiatrist, but also the mother learned some exercises so that Arthur can practice at home. It also does swimming and, like most of the children with this deletion, Artur loves water. "It's something that is not explained scientifically. " For the 1p36 deletion there's no cure, as noted by Juliette Dupont only therapy to improve quality of life. Some children can get some independence, and there are even cases of adults who come to get to work.  However, dependence on third parties is inevitable for all life. Raquel does not work, but
think in getting back to work next year and put Artur in a kindergarten. "Some children with this deletion
able to follow the teaching, "and this is what she thinks that can happen to the child, because Artur seems to understand everything. Just have difficulty expressing himself.
Without knowing anyone in Portugal with the same problem Raquel seeks support in the association
Unique. Paula Brito e Costa, from Raríssimas considers essential for families to share experiences with others
with the same experience.

Today on a portuguese newspaper is an article about Artur. They have a small summary of the article on the web page: http://www.dn.pt/inicio/portugal/interior.aspx?content_id=1832280 As soon as I can I'll put here a scan of the newspaper article and translate it for you. ;)
Thank you all for following this blog.
I have to say that Artur has a great medical team which is one of the best things. Many people complains about health in Portugal but I have nothing to complain about except one small thing in the last time he had to go to the emergency of the hospital. From doctors to all medical staff they all have been excellent.
This blog is in portuguese and english because I've meet a group of people in Facebook that have children or family members with 1p36 and we have become a family. When I started this blog I could not keep them apart from this so I publish also in english so they can also understand it.

Nova convulsão - new seizure

Yesterday Artur had a new seizure, I had to call 911 because it wouldn't stop even after I gave him is SOS medication. When the ambulance arrived  the seizure was starting to stop. When the doctors arrived the seizure has already stopped, but they decided it was better to go to the hospital. He spent there a few hours and the doctor said he needed a new dosage of one of the medications. For the first time I was unhappy with the way they handle Artur in the ER... Well I guess it can't be only good experiences when he goes to the hospital...

2 anos - 2 years

O Artur faz hoje 2 aninhos!

Artur's birthday today! He is 2 now, not a very good day so far, he has fever. Because of his fever he has small seizures that take less than 2 minutes to pass... later on I'll post a picture of today.

Otite - Ear infection

Artur 28-02-2011

Artur 01-03-2011

We went to the hospital with Artur because he was scratching the right ear a lot. When I touched his ear he winced.  Well, its an ear infection! He is starting the treatment and soon will be just fine.
I have to say it: Damn teeth!

terça-feira, 1 de março de 2011

mas que dia... - what a day...

Today Artur have been impossible to handle, cause of two teeth that are coming out. With all the pain it causes he has spent all morning crying and screaming. Sometimes when I give him something to chew he calms down a little bit, but he lost his appetite.

Olá a todos! - Hi everyone!

Well an update was in order because... suspense... :P
Artur started saying NO! (in portuguese it's não but he says nhão or only ão)
When I try to put is jacket on he says no and he just doesn't let me! he bends his arm, grabs the sleeve and doesn't let me do a thing! If I try to sit him in the sofa he says no and throws himself to the side so that he can be just laying there!

Who rocks? Artur!

Consulta neuro-oftalmologia

Ficou a parecer o gatinho do Shrek! hehe

Artur went to his neuro-ophthalmology appointment today. He was really grumpy and sleepy, so it was hard to do the eyes exams. They scheduled a new appointment to August, for now they said his seeing ability is below the normal to children with the same age. They did a exam that they put some drops in Artur's eyes to dilate the pupil of the eye. He looked like the Puss In Boots from Shrek! hehe

23 meses - 23 month old

O Artur fez ontem 23 meses!

Artur turned 23 month old yesterday! He is so big already, and time flies by so quickly. Monday he has an neuro-ophthalmology appointment to see if everything is OK with his eyes. Some 1p36 children have some eye issues and Artur sometimes seams to have one eye in a wrong position, so its better to see a doctor now that to wait and if he has a problem it could get worse. he follows objects really well and he only does it sometimes when he looks up.

O desenvolvimento do Artur\ Artur's development

Artur was born with 3,440kg (7,58lb) and 49,5 cm (19,49 in).
At 6 months he started to roll-over first just to one side and later to both sides.
Had his first tooth he was 7 months. But was since 3 months with his gum swollen and giving him bad time.
Around 9month old- 1 year old he started grabbing objects and transfer them from one hand to the other. This was a big win because for 1p36 kids its hard to grab objects and they often pay more attention to humans that to objects.
Around 1 year old said his first word- mum and started having some head control.
He did one hearing test (evoked potentials???) and as a result we were told he had moderate hearing loss in both hears, we are waiting for the hearing aids that the hospital will give Artur, will be 1 year of waiting in May...
15 month had his first seizure with fever, and was in the hospital for 9 days, at 18 month the 2nd seizure also with fever and was in the hospital for 4 days, after this seizure started taking one med for seizures. The 3rd seizure was at 19 month old and he had no fever and was on the hospital for 2 days. Last January (22 month) had his 4th seizure spent a night on the hospital. The epilepsy was diagnosed on the 3rd seizure he is taking meds for it but being a baby its hard to keep it well dosed cause they are growing and gaining weight and off course there is bigger chances of having a new seizure like the last one due to need a new dosage of medication.

About weight and height, 1p36 children have some difficulty in gaining weight and in getting bigger, so Artur has no percentile in weight and in height its around the percentile 5. (don't know if that's the same in US and other countries)

When the genetics doctor talked to us, she took us to be tested to see our karyotype and to see if one of us might have caused it. We decided as a couple not to blame the other or ourselves if one of us might have caused it. When Artur was 6 months old we got the result by mail to our home where we could read the name of his disease: 1p36 deletion syndrome. I'll try to explain it in a easier way, Artur has some genetic material missing in the short arm of the chromosome 1 (we have a pair of each chromosome). It might cause lots of implications on a physical level, and on other level of his development. Our results came up and we realised none of us caused it, it was an error when both female and male cells got together to form the one cell that became an embryo. About Artur, now with almost 2 years old we know right now he has some delay on the motor development, he was born with axial hypotonia and he started physical therapy when he was 2 month old. Yes he shows improvements because of the PT, he had no head control when he started it and now he has it, not yet perfect but he can hold his head weight. He can't sit up yet with no support, he says mommy (mamã), must times only má... lol Also says hi (óá instead of olá) and he is grumpy and stubborn. ;) I guess he took from me the grumpiness and stubbornness. :P

O meu bebé. My baby

Artur was born on 17-03-2009. It was a normal pregnancy and he was born by c-section after the labour was induced cause I was already on my 41week and 4 days, had to be a c-section cause Artur didn't want to come out. ;)
At his 3rd day the doctor did some blood tests and found out he had an infection and he had jaundice. So we had to be in the hospital to take antibiotics for the infection and do make the jaundice treatment. Since he had a large fontanelle (soft spot) they did a echo to his brain and decided to do all sort of exams to see if everything was ok. One of the doctors they called was the genetics specialist that did some blood exams like karyotype and metabolic diseases. On the day they send him home (he was already 15 days old) the genetics doctor told us he had a deletion on the chromosome 1 but not really the name of the disease cause they were still completing the karyotype study... (to be continued tomorrow...)
